To best train or coach you, we want to take the time to get to know you a little better.  Please help us by completing our simple, 2 page form below, consisting of 7 questions. The questions focus on your lifestyle and health history as they relate to fitness.  We will talk more about your answers at your Kick-Off Session, Step 3.  However, don't forget to advance to Step 2 to purchase your package, after completing this form.
